>> After downloading the app, click on ‘EPFO’.

>> Select ‘Request for Advance (COVID-19)’.

>> Enter your ‘UAN’. Click on ‘Get OTP’. After that submit ‘OTP’ and click on ‘Login’.

>> After logging in successfully, enter your bank account number and select ‘Member ID’ from the drop down menu. Click on ‘Proceed for claim’.

>> Enter your ‘Address’ and click on ‘Next’

>> Select ‘Form 31’ from the drop down menu, fill the amount required in ‘Amount of Advance Required’ and upload the photograph of your check book in ‘Check Image’. Your name, bank account number and IFSC should be clearly visible in the check details. The photo should be in jpg or pdf and the size of the photo should be up to 100kb.

>> Check the declaration box and click on ‘Submit OTP’ button. After receiving the ‘Aadhaar OTP’, click on the ‘Submit’ button. Your COVID-19 Withdrawal Form has been successfully submitted.
